How to Get a Solo Parent ID in Cagayan de Oro City

 

PROSESO SA PAG-APPLY SA SOLO PARENT ID 

(FOR ONLINE APPLICATION) 

1. Bisita sa https://services.cagayandeoro.gov.ph/soloparent/ 

2. I-fill-up ang application form 

3. I-upload ang gikinahanglang mga dokumento 

• 1x1 Photo ID
• Brgy. Certificate of Residency
• Sworn Affidavit (Unwed, Abandonment, Separated by fact) (if applicable) - sa Public Attorneys Office (PAO) sa may Tiano Brother's Street ni makuha.
• Order of finality (Legally Separated, Annulment) (if applicable) 
• Death Certificate (Widow/Widower) (if applicable) 
• Medical Certificate (If the spouse has physical/mental incapacity) (if applicable) 
• Birth Certificate of the minor Child/Children 
• House Sketch 
• Photocopy of ITR or any proof of income (If employed)
• Specimen Signature (White background) 

4. Maghulat og "reply" sa system kung walay kakulian sa imong application 

5. Maghulat og sulod sa 30 ka adlaw para makuha o’ mahibaloan kung pasado na matagaan sa Solo Parent Identification Card 

Note: 

• Ang Solo Parent Identification Card valid lamang sa usa ka tuig apan renewable kini.
• Pwede ra pod mag bisita sa opisina sa CSWD (City Social Welfare & Development) naa sa City Hall.
